Secondary Stroke Prevention Education Tool
Introduction/Problem
Results/Progress to Date
It was identified that there was not a standardized process of education for
stroke patients and their caregivers/family. The need for a process of education
that was efficient, effective, and done in a timely manner was identified. A group
of key stakeholders from the NIMU and Farr 11 was formed with a goal of
developing a standardized stroke education process including the tools and
resources to support meaningful, progressive and ongoing education allowing
for consistency and continuity as patients progressed across the continuum of
care from admission to discharge.
Aim/Goal
The aim/goal of this project was to facilitate a standardized education process
for the stroke patient. The long term overreaching goal was to improve patient
outcomes post discharge and beyond.

The Interventions
Developed of a Secondary Stroke Education Teaching Tool
Developed of systems to support effective implementation of the tool
Educated staff regarding the use of the Secondary Stroke Education Tool
Implemented real-time coaching and feedback for staff at the bedside
Implemented real-time auditing and a continuous quality monitoring
program.
Lessons Learned
Challenges were encountered with the implementation of the tool given the current
confines of the hybrid charting system. Engagement of frontline staff I n
consideration of these challenges , and identifying and piloting potential solutions
was a key component to our success
The Secondary Stroke Education Teaching Tool has standardized the delivery of
Stroke education and has streamlined the documentation thereby creating a more
efficient, fluid, and meaningful process of patient and caregiver/family education
Opportunities remain to better demonstrate how teaching is individualized to each
patients specific risk factors
Next Steps
Leverage and utilize current technologies to facilitate transition of the tool to
electronic documentation systems
Revise the tool to more clearly demonstrate how education is tailored to
each patients individual risk factors and learning needs
Develop systems to facilitate the collection and analysis of data to determine
the impact of this work on post discharge outcomes and identify further
opportunities for improvement
